Pourquoi la dentisterie numérique va changer notre quotidien ?

La dentisterie vit une véritable révolution numérique. Il va donc falloir apprendre à travailler avec de nouveaux outils qui sont, du reste, déjà des outils actuels. Cela représente certes des difficultés, mais aussi un accomplissement pour le praticien et une satisfaction pour le patient. Que s’est-il passé en l’espace de 50 ans ? La dentisterie a été révolutionnée par les implants et l’adhésion. Désormais, c’est l’heure de la révolution CFAO.
Jusqu’à une date récente, nous utilisions des techniques artisanales à 100 %. Aujourd’hui, la CFAO envahit les laboratoires. C’est en premier lieu le scannage de modèles en plâtre pour une dématérialisation numérique qui permettra de travailler à l’aide de logiciels et non plus avec ses mains. Plus de spatule ni de cire, mais un travail propre pour obtenir des infrastructures d’un même niveau de qualité avec un gain de temps et une plus grande fiabilité. La CFAO constitue la seule technique qui permette aujourd’hui de travailler les armatures en homothétie de façon à maintenir une épaisseur constante et suffisante à la partie cosmétique, garante de résistance. Il est possible d’usiner tous les matériaux : le titane, la résine, le zirconium. On est aujourd’hui à même de réaliser armature et cosmétique en CFAO. Cette technologie ne change rien pour nous au fauteuil. C’est en revanche au laboratoire que cela va beaucoup évoluer : les armatures seront réalisées en CFAO et le prothésiste pourra exprimer son talent dans la réalisation cosmétique.
Schématiquement, la CFAO s’appuie sur un logiciel dont une partie permet la conception et l’autre la fabrication à l’aide d’une machine outil qui usine le matériau choisi.
Elle a tout son sens en implantologie pour :
• les piliers anatomiques : si le berceau gingival a été organisé, il sera respecté par le profil d’émergence du pilier en zircone ;
• la réalisation des faux moignons et des armatures pour les couronnes en deux étages ;
• les couronnes en un étage.
Plus le nombre de piliers implantaires est élevé, plus les techniques artisanales sont limitées. En effet, avec les techniques de coulée, et même après soudage, il est très difficile d’obtenir des armatures passives. En revanche, avec des empreintes conventionnelles et le scannage du modèle, on obtient des armatures en titane d’une précision absolue.
De quels outils numériques disposons- nous au cabinet dentaire ? En premier lieu la radiographie et, ensuite, les appareils photo. Ces derniers constituent de véritables outils de communication : ils facilitent la compréhension de nos patients et, avec des logiciels de présentation tels que Keynote ou PowerPoint, via Skype, nous échangeons avec notre laboratoire sur les questions d’esthétique.
Dans un futur proche, c’est l’empreinte numérique qui va se développer. À l’heure actuelle, nous utilisons des élastomères dans des techniques un temps ou deux temps deux viscosités. Mais à l’interrogation : « Est-il possible d’aller vers le numérique ? », il est répondu « Oui » à 61,4 %. Dès lors, les questions qui se posent désormais sont : Quel système adopter ? Est-ce le moment ?
En réalité, la nouvelle technique doit faire aussi bien tout en étant aussi simple. Il faut aussi vaincre le poids de nos habitudes, car changer de méthode demande un réel effort pour maîtriser ces nouvelles techniques.
L’empreinte numérique présente divers avantages :
• le capteur est unanimement vécu comme plus confortable que les techniques classiques ;
• la technique du double cordonnet est commune aux deux méthodes afin d’enregistrer le profil d’émergence ;
• lorsqu’une zone est mal enregistrée, en numérique, seule la zone mal définie est à nouveau enregistrée contrairement à l’empreinte classique, laquelle doit être refaite entièrement ;
• les niveaux de précision du joint dento-prothétique sont similaires.
Une valeur de 80 μm à 100 μm est acceptable, et le numérique offre 60-70 μm.
Deux bémols cependant sont à prendre en compte :
• l’empreinte des logements de tenons visant à réaliser des reconstitutions corono-radiculaires commence seulement d’être possible ;
• l’enregistrement numérique ne permet pas d’enregistrer aussi loin dans le sillon gingivo-dentaire.
Il existe aujourd’hui une pléthore de systèmes qui donnent des résultats très voisins. Un critère de choix pourrait être l’encombrement et le poids (de 300 g à 900 g) de la caméra et l’ergonomie du logiciel. Quant au poudrage, il n’est plus nécessaire, par exemple, dans le système 3 Shape Cure®. Des modèles d’une précision comparable au plâtre sont obtenus par stéréolithographie ou usinage. Le système Encode® est extraordinaire, mais il faut alors passer à une chaîne entièrement numérique. La caméra permet aussi l’enregistrement de l’occlusion sous une incidence latérale, de façon très simple.
Il s’agit donc d’un réel outil du futur, un véritable allié qui n’a pas de limites. Avec lui, nous nous dirigeons vers la virtualisation de nos patients dont nous créerons des avatars…
